If I had to use the models you listed and nothing else:
Convert the berserkers and chaos marines into plague marines make a 5 man and a 6 man team
Keep ten terminators in two five man teams
Convert six terminators into two three man obliterater teams
Keep the rest as is.
Attach lords to the terminators squads then deep strike.
You will have a lot of 2+ armor and T5 with mark of nurgle but a very low model count.
