There's a paragraph in the rule book that says in certain circumstances there could be a -1 or -2 modifier on a morale test but it never elaborates on what would constitute such a modifier. What exactly would cause a morale modifier?

Most of those might be under the codexi. The tyranid Devourer does and on page 44 of the BRB- C) Losing an Assault gives another example of moral modifiers.

The beacon on Space Marine Landspeeder storms - when the transported scouts assault out of it.
Losing an assault.
Certain weapon effects.
Certain wargear and or abilities (Mephiston has one that makes characters in btb with him take a check at -4 - if they fail, he gets to reroll hits and misses.)
