i know they can ingame but in fluff his staff makes u instantly die of pain but plague marines cant feel pain so how can it affect them

They feel NO pain, because Papa Nurgle protects them from it. They simply don't feel it, or feel it so weak that they ignore it.

Or their bodies go into super defence mood and they just murder themselves with the rapid mutation regeneration and endorphins.

Whether they register the pain is a different matter..

Yup, but they don't care about pain of damage anyway. They are so-fkin'-tough that you can slash, hit, burn, break, crush and anything else, and they will just smile, because you'll get infected. BTW, how does Bile kill 'em in-game? Instant death?

his staff causes instant death to who ever is wounded by it

bile causes instant death.

They may not feel pain but that could be due to a dulling of senses, they can still be killed, and mutated, they just wouldn't register the pain, kinda like an archo-flagellent
