I am a bit confused by the wording is it D3+3" blast or a Blast template + D3+3".

It's a blast that has a RADIUS of d3+3". So it's an 8", 10", or 12" blast.

it is D3+3". So it's either a 4,5,or 6" Radius blast.

I will try to explain this as clearly as possible:

You have a circle, which is the blast.

This circle has a radius, which is the distance from the center of the circle to the outside.

In the case of the death strike this distance is D3+3, making it 4, 5, or 6 inches.

That means that the total diameter (distance across) of the blast is 8, 10, or 12 inches.
